On Tuesday 07 January 2020 between the approximate hours of 12:15pm and 1:30pm Juan Tafoya walked away from the lobby in the area of 1500 East Osborn Road in Phoenix, Arizona while waiting for a medical appointment.  He suffers from a mental health condition which may cause him to be confused and easily gets lost.

He was last seen wearing a black or gray sweater, dark color pants and sandals.

He is described as a 60 year old Hispanic male being approximately 5'5" tall; weighing approximately 160 pounds with brown hair and brown eyes.
